  • HealthBridge accepts most major health plans, including Blue Cross Blue Shield, Blue Care Network, Priority Health, Medicare Advantage plans, and traditional Medicare Part A. Please feel free to call and speak with our experienced Case Management staff to discuss your health plan.

  • What is Post-Acute Rehabilitation?

  • After an illness, injury, or surgery many patients are not able to go right home from the hospital due to weakness or the need for continued therapy and strengthening. HealthBridge Post-Acute Rehabiliation provides physical, occupational, and speech therapy along with nursing and physician services to help you on your path back to your home.

  • What should I expect when I admit to HealthBridge?

  • HealthBridge is a short-term post-acute facility. Meaning that your stay here starts with your discharge plan back to your home. You will meet with our interdisciplinary team to develop a care plan that will help you reach your goals.

  • What should I bring during my stay?

  • We want you to be as comfortable as possible while you are going through your recovery. You will be participating in therapy each day so make sure to bring some comfortable clothes and shoes that you can move around in. Also, any personal care items that you may prefer to use.

  • How will I know when I am ready to go home?

  • Our experienced interdisciplinary team will meet with you weekly to review your progress and help you plan your discharge back to your home.
